Breast Reduction on an elderly patient with free-nipple graft technique.  Usually in breast reduction the nipple/areola is left attached to the underlying breast gland.  But in cases like these, the vital connection to the nipple would be too long and the blood supply questionable.  So FNG technique involves removing the nipple completely, creating a nicely shaped breast mound and grafting (like a skin graft) the nipple in the ideal position.  For truly huge reductions with a great deal of nipple elevation, it is the preferred technique.
